Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above in B103 changing table was unclean and diaper genie had fecal matter on it which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in 2 of 4 staff memebrs did not have a Mandated Reporter certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Staff #1 did not apply diaper cream to child per parent request, to prevent diaper rash. This is a potential health, safety and personal rights risk to children in care.
Responsibility for Providing Care and Supervision — Based upon interviews with staff, staff did not see the incident that occurred when a child was hurt and did not remove the hazardous item in time to avoid the injury which poses a potential health, safety and personal rights risk to children in care.
Criminal Record Clearance — Based upon record review and Director statement, two subs were not associated to the facility which is a potential health, safety and personal rights risk to children in care.
