During the staff file review, the LPA reviewed an expired Mandated Reporter certificate for S2. Licensee stated S2 does not have a current Mandated Reporter certificate on file.
Child's Records — Based on the record review and interview, the Licensee did not meet the above regulation which poses a immediate Health, Safety & Personal Rights risk to the children in care. During the children’s file review, the LPA was unable to review a file for C1. Licensee stated the file for C1 could not be located.
Mandated reporter training — Based on record review the licensee did not comply with the section cited above in the licensee was unable to provide proof of the renewal mandated reporter training for mandated reporter after expiration on 06/30/2025, which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization — influenza, pertussis and measles — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During review of the staff files LPA was unable to review immunizations for S1.
