Alterations to Existing Building or Grounds — Based on observation, interview, and record review, the licensee did not comply with the section cited above in that a garage was converted for day care use and no notice was given to the San Diego Regional Office, which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation and record review, the licensee did not comply with the section cited above in that one (1) infant under 12 months of age has no LIC9227 on file which poses/posed a potential health, safety or personal rights risk to persons in care.
