Reporting Requirements — Based on LPAs interviews and records review the facility did not place a call to the department to report construction taking place at the facility on an Unusual Incident Report (UIR) which posed a potential health, safety, or personal rights risk to the children in care.
Fixtures, Furniture, Equipment and Supplies — Based on the observation, the Licensee did not meet the above regulation which poses an potential Health risk to the children in care. During facility tour the LPA observed two solid waste containers that did not have tight fitting lids.
Mandated reporter training — Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During file review it was discovered S1 did not have a current Mandated Reporter certificate on file. The certificate on file expired on 2-21-2024.
