Licensee was also unable to find S1's file and only had her negative TB screening on record. S1 is fingerprint cleared and associated but is missing her immunizations, current FA/CPR certificate, LIC 9108, and LIC 9052 which poses/posed a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — Based on observation the garage, living room, and kitchen were missing barriers and mechanisms prohibiting access to those unlicensed areas when LPA first arrived for inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
