Teacher Qualifications and Duties — Based on records reviewed and interviews conducted, the licensee did not comply with the section cited above in that S3 was not a fully qualified teacher and was left to supervise an aide on multiple occasions, which poses a potential health, safety or personal rights risk to children in care.
Teacher Aide Qualifications and Duties — Based on LPA observation and staff interviews, the licensee did not comply with the section cited above in 21 children were present with the director and one unqualified aide. Three (3) out of 21 children were napping on their cots. The director stated that (S1) was on break and would be returning shortly. (S1) returned approximately 20 minutes later, which poses a potential health, safety or personal rights risk to children in care.
Activities/Napping — interviews conducted indicating that on more than one occasion, one staff member has cared for more than 24 napping children. This may pose a threat to the health and safety of children in care.
Teacher-Child Ratio — at least two staff members on separate occasions providing care for more than 12 children, per interviews conducted. This may pose a threat to the health and safety of children in care.
Limitations on Capacity and Ambulatory Status — children under the age of two and over the age of 5 being supervised and/or cared for at the facility during operating hours, per interviews conducted. This may pose a threat to the health and safety of children in care.
Personal Rights — a child's food/drink being taken out of their lunch box and consumed by staff members and/or other children. Staff member has breast fed her own child in the presence of children in care. This may pose a threat to the health and safety of children in care.
