Observation of the Child — Based upon staff interviews, at least 1 child was left in soiled clothing for extended period of time which posed a potential health, safety and personal rights risk to children in care.
Fixtures, Furniture, Equipment and Supplies — Based on interviews, it was determined that the facility did not have running water on 3/5/2025 & 3/6/2025. Facility closed on 3/5/2025, however the next day 3/6/2025, there was insufficient water pressure for handwashing or to flush the toilet.
Reporting Requirements — Based on interviews, licensee failed to report to the department that the facility closed down on 3/5/2025 due to a broken pipe resulting to insufficient water being delivered to the building. Facility also did not have sufficient running water part of the day on 3/6/2025.
Responsibility for Providing Care and Supervision — Based on observation, LPA observed 2 children without visual supervision in the bathroom. Supervising teacher was observed on the other side of the classroom.
Teacher-Child Ratio — Based on record review and Ms. Kurtenbach's statement, the Aide in the Grizzlies did not meet the requirement for section 101216.2(d), she only has 3 units completed.
The maximum group size with two teachers, or one fully qualified teacher and one aide shall not exceed 12 toddlers. — Based on observation, there were 14 toddlers with 1 fully qualified teacher and 2 aides present today.
