Notice to parents that site visit reports are available — Based on record review and interview, the licensee did not comply with section cited above. LPA J. Culbert observed that eight students did not have a LIC 9224 in their file. When licensee was interviewed they discloed that they did not know they had to give the report to new parents.

Health-Related Services — At the facility, LPA observed that Director stored medication for Child2, Child3, Child 4, and Child 8. During file review, LPA discovered that thier files were missing written instruction and approval from parents for administering the medication. This is a potential health and safety risk to persons in care.
