Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in staff one did not have physician report on file at time of inspection. This poses a potential health, safety or personal rights risk to persons in care.
Food Service — Based on observation and interview, the licensee did not comply with the section cited above in there was two expired frozen milks, one in the refridgerator and one in the freezer at the time of inspection, photos taken, milk frozen unable to smell for odor. This poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on interviews and observations, D has failed to prevent C1 from causing injuries to children in care, C1 continues to flip tables, throw chairs, bite and scratch other children. This is a potential health and safety risk to the children in care.
Plan of Operation — Based on record review LPA observed that the policy for unacceptable behavior also known as the “three strike policy,” was not being followed and when C1 has behavioral issues the child is taken to the director and provided a cookie, walks etc.
Personal Rights — Based on confidential interviews C1 becomes very physical when upset hurting and hitting other children in care. C1 has caused red marks, bite marks, and other minor injuries to children in care
Observation of the Child — Based on three of three interviews parents were not recieving injury reports regarding their children. C1 is a very physical child who causes red marks, bite marks and other minor injuries to children in care.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
