Reporting Requirements — Based on interview and record review, the licensee did not comply with the section cited above in that the facility did not report an incident on 7/15/2024 where a facility van damaged a fence and the door of classroom 104. This poses/posed a potential health, safety, or personal rights risk to persons in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in that there were 3 metal panels accessible to children in the playground. When opened there was a label on the inside of the panels that stated "Danger hazardous voltage contact will cause severe injury or death." This poses/posed a potential health, safety or personal rights risk to persons in care.
Licensing review limited to health and safety — Based on record review the licensee did not comply with the section cited above in that the licensee did not maintain a copy of verification that the facility director has completed an orientation given by the department, which poses/posed a potential health, safety or personal rights risk to persons in care.
