Fixtures, Furniture, Equipment and Supplies — This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above in that play structure is not in good repair in several areas, plastic coating was missing on play structure exposing metal parts of the play ground. The slide surface is also broken which poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation and interview, the licensee did not comply with the section cited above in that the rubber flooring/surface in the outdoor area by play equipment surface is not in good repair, which poses/posed a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation and interview, the licensee did not comply with the section cited above in that play structure is not in good repair in several areas, metal flooring was observed plastic coating to be missing, with areas having raised pointed parts. Also at least two nuts or bolts were missing. In addition a slide surface is broken, which poses a potential health, safety or personal rights risk to persons in care.
