Safety precautions shall include but not limited to: (3) Where children are less than five years old are in care, stairs shall be fenced or barricaded. Based on observation, there were 8 children present out of which six of them were under the age of five and the stairs were not barricaded, therefore Licensee did not comply with the regulation cited above which poses a potential health, safety or personal risk to persons in care.

Mandated reporter training — Based on observation, interview, and record review, the licensee did not comply with the section cited above in renewing the mandated reporter tarining certification which poses a potential health, safety or personal rights risk to persons in care.
