Operating without a license — ; Based on observation on June 26, 2026, Facility had 4 school-age children present in the home. LPA observed no desks or curriculum being provided. Children were observed sitting on the couch. Today, LPAs was denied entry into the home. During Today's visit LPAs observed an infant child being dropped off to the facility and entered inside and a school age child at the front screen gate. This poses an immediate risk to the health, safety, and personal rights of children in care.

Family home — children supervised at all times — two children wandered away from staff and were unsupervised which is a potential risk to the health and safety of children in care.
Staffing Ratio and Capacity — during the visit there were 5 infants in care which poses a potential risk to the health and safety of children in care.
Infant Safe Sleep — 2 infants were sleeping with items in the play yard which is a potential risk to the health and safety of children in care.
