Notice to parents that site visit reports are available — LPA's record review and the Directors acknowledgement that the report containing the Type A citation and the LIC 9224 form were not provided to parents/auth. representatives as instructed, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — LPA Claytons observation of a collapsable wagon overflowing with trash (open bags containing discarded food, soiled diapers, etc.) on the play yard and accessible to children in care, which posed an potential Health and Safety risk to children in care.
Buildings and Grounds — LPA Claytons observation of a can of Comet cleanser on the play yard, a can of house paint in the classroom as well as Lysol disinfectant wipes and Lysol spray throughout the campus, all accessible to children in care, which posed an potential Health and Safety risk to children in care.
Reporting Requirements — During LPA interviews with staff and Licensee, it was reported that on 03/13/2026, 2 (two) children in care hit one another in the face with a frisbee and one of the children sustained a black eye. The incident was not reported to the Department as directed in the above Regulation, which poses a potential Health and Safety or Personal Rights risk to children in care.
Indoor Activity Space — Based on observation, the licensee did not comply with the section cited above in keeping classroom carpets cleaned, which poses/posed a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in 1 out of 3 toilet seat not being properly secured, which poses/posed a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in making sure back outdoor gate remains closed, which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 5 out of 5 current certificaiton, which poses/posed a potential health, safety or personal rights risk to persons in care.
101700.3(b)(1), A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. Based on observation, interviews, and review of lead water testing samples taken on 09/16/2023 revealed that levels of lead were beyond allowable amount deemed by the State of California-5 water outlets identified (B and C).
