Responsibility for Providing Care and Supervision for Infants — Based on observation, interview, record review, the licensee did not comply with the section cited above in that the documentation recorded for infants safe sleep did not include position infant was sleeping in or infant's condition which poses/posed a potential health, safety or personal rights risk to persons in care.
Modifications to Infant Needs and Services Plan — Based on interview and record review, the licensee did not comply with the section cited above in that eleven (11) out of twelve (12) infants Needs and Service Plan had not been updated quarterly which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff-Infant Ratio — Based on observation and interview, the licensee did not comply with the section cited above in that for approximately 10-15 minutes there was only one staff member with six (6) infants which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Food Service — Based on observation and interview, the licensee did not comply with the section cited above in that six (6) infants bottles and food only contained a name label and not a date which poses/posed a potential health, safety or personal rights risk to persons in care.
