Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above for one infant (C1) which poses a potential health, safety or personal rights risk to persons in care. LPAs observed child in a play yard covered with a blanket.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above for herself which poses a potential health, safety or personal rights risk to persons in care. Licensee has not renewed her mandated reporter training.
Family home — preventive health training, including pediatric CPR and first aid — Based on record review and interview, the licensee did not comply with the section cited above for assistant, S2 which poses a potential health, safety or personal rights risk to persons in care. S2 was observed alone with children but has not completed the Pediatric CPR/First Aid training.
Alterations to Existing Building or Grounds — Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPAs observed day care children napping in master bedroom which is off limits to children.
Staff immunization — influenza, pertussis and measles — Based on record review and interview, the licensee did not comply with the section cited above for assistant, S2 which poses a potential health, safety or personal rights risk to persons in care. Assistant does not have the required immunization's in file for measles, pertussis and influenza/ statement declining influenza.
Immunizations — Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPAs observed that Licensee has not maintained a file with the required forms for child C3. Child C5 is missing their immunization record and children, C4 and C6 are missing the Parent's Right's form (LIC 995).

Infant Safe Sleep — Based on observation, interview and record review, the licensee did not comply with the section cited above for three infants (C1, C2 and C3) which poses a potential health, safety or personal rights risk to persons in care. Licensee has not maintained a 15-minute nap log for all infants in care.
