Infant Safe Sleep — Based on the records review and interview, the licensee stated that she did not know that she needs to document her checks on infants so she did not have a sleeping log, which poses/posed a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on the interview with the licensee, the licensee did not have immunization records for kids in care, which poses/posed a potential health, safety or personal rights risk to persons in care.
