Immunizations — Based on record review, the licensee did not comply with the section cited above in two of six child files audited. Infant Safe Sleep — Based on observation, interview and record review, the licensee did not comply with the section cited above in two of two infants observed during nap. Where Child 3 had a bottle in the crib that was propped while C3 was asleep. Child 1 was utilizing a sleep sack during nap without a medical exemption on file. Immunizations — Based on interview and record review, the licensee did not comply with the section cited above in four of eight child immunizations audited. Where childrens immunizations were either not updated, available or reflecting childs name and DOB. Which poses a potential health, safety or personal rights risk to persons in care.
Family home — safe toys and play equipment — Based on LPA observation, the licensee did not comply with the section cited above in that the cover for the springs on the trampoline is worn which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on LPA observation, the licensee did not comply with the section cited above in that the play pen has a loose sheet on the mattress which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that the mandated reporter training expired in 08/2023 which poses a potential health, safety or personal rights risk to persons in care.
Criminal Record Clearance — Based on record review, the licensee did not comply with the section cited above in 1 of 1 staff reviews revealed staff did not have documentation of criminal clearance which poses a potential health, safety or personal rights risk to persons in care.
