Buildings and Grounds — Based on the interviews and information obtained Facility did not have operable toilets for the staff and children . Children had to walk to the different facility located in the different address to use the restrooms .
Reporting Requirements — Based on the information obtain Facility had a maintenance issue and did not have operable toilets for the children on 04/22/2024.This unusual incident was not reported to the Department .
