Responsibility for Providing Care and Supervision — Based on three of three interviews and records reviewed, C1 was left alone in an outside restroom for approximately fifty (50) minutes without supervision. C1 was found by D at the time of pick up after searching the facility with C1’s guardian, which poses an immediate health, safety and/or personal rights risk to children in care.

Outdoor Activity Space — Based on observation and interview, the licensee did not comply with the section cited above. LPA observed five large holes in the unlevel hills of the playground. Due to weather the ground is erroded and the holes are repeatedly exposed. Which poses a potential health, safety or personal rights risk to persons in care.
