This requirement was not met as Based on interviews conducted 6 of 6 staff members confirmed that P1 stated they brought C1 into the classroom from the hallway where they were alone. Witnesses interviewed stated they found C1 in the hallway alone and took C1 back into their classroom. This is a potential health and safety risk to the children in care.

This was not met as evidenced by, Based on interviews conducted it was disclosed that infant classroom would be out of ratio until 9am when another staff memeber arrived or for the whole day due to lack of staffing. Responsibility for Providing Care and Supervision for Infants:(a)In addition to Section 101229, the following shall apply: (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This was not met as evidenced by, Based on interviews conducted it was disclosed that due to being out of ratio and having different staff members in the room, children were getting hurt more often and adequate care and supervision was not happening. This is a potential risk to the health and safety of children in care.
Infant Care Center Sleeping Equipment — Based on observation, the licensee did not comply with the section cited above in crib sheets were either too lose or too tight causing mattress to bend which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 2 out of 4 persons missing immunizations in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher Qualifications and Duties — Based on observation and record review, the licensee did not comply with the section cited above in 2 aides were left alone with the children while outside which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 2 out of 4 persons missing Mandated Reporter certificates in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Needs and Services Plan — Based on record review, the licensee did not comply with the section cited above in 4 out of 8 persons missing LIC9227 in file which poses/posed a potential health, safety or personal rights risk to persons in care.
