Family home — safe toys and play equipment — LPA obtained information on an infant in an exersaucer. Excersaucers are a prohibited item for Family Child Care Homes (FCCH’s). However, an infant was seen in what appears to be a lime-green, blue and white excer-saucer, with dates going back to January 2025. This poses an immediate health/safety risk to children in care.

Personal Rights (a)Each child receiving services from a family child care home shall have certain rights that shall not be waived/abridged by licensee regardless of consent or authorization from the child's authorized representative. These rights include, but are not limited to, the following: (2)To receive safe, healthful, and comfortable accommodations, furnishings, and equipment. LPA obtained information on multiple children sleeping in various inappropriate items and furniture equipment within the daycare.

Personal Rights — Based on information obtained during interviews and records reviewed, it was determined staff did not take sufficient steps to prevent a child from biting other children over the course of approximately five to six months which poses a potential personal rights risk to children in care.

Family home — in-ground pool drowning prevention requirements — Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure drowning prevention and pool equipment inspection logs were completed which poses a potential safety risk to children in care.

Child's Records — Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure records were maintained and available for review for two out of four children present which poses a potential health, safety, and personal rights risk to children in care.

Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above in LPA observed the pool fencing post on the west end of the outdoor area to be detached and unsecured, a section of the mesh panel to be leaning inward and the entry panel/gate does not self-latch to close shut which poses a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in the facility was unable to provide proof of 15 minute log for an infant under 2 years of age, which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in the licensee and staff member did not have proof of the required mandated reporter training, which poses a potential health, safety or personal rights risk to persons in care.

Family home — admission procedures and parent rights — Based on record review, the licensee did not comply with the section cited above in 4 of 9 children present and in care did not have the lIC995A Parent's Rights forms on file which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section staff present and caring for childen did not have proof of immunization records for measles, pertussis and influenza or letter to decline the influenza. In addition, the staff do not have proof of screening for Tuberculosis, which poses a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above in 2 of 9 children were missing the required LIC700 Identificaiton and Emergency cards and all 8 of 9 children present were missing the LIC627 Medical Consent forms which pose a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review, the licensee did not comply with the section cited above in 2 children present and in care, did not have proof of immunization records which poses a potential health, safety or personal rights risk to persons in care.
