Mandated reporter training — Based on staff record review which revealed L1, S2, and S3 did not have a current AB1207 mandated reporter training. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on LPA's staff record review, S2 records were missing evidence of immunity against measles, pertussis and Tuberculosis. Additionally, LPA's staff record review indicated S3 records were missing evidence of immunity against pertussis and Tuberculosis. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation and interview, the licensee stated she was not aware that sleep logs were required for children over 12 months old and was unable to provide 15 minute sleep logs for C4 and C5. The licensee did not comply with the section cited above for two out of two infant files reviewed during today's inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
