All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing, or volunteering in a licensed facility...Request a transfer of a criminal record clearance as specified in Section 102370(j) This requirment was not met as evidenced byUpon arriving at 10:15AM, LPA's observed Staff #1 working at the facility prior to Licensee requesting a transfer of a criminal record clearance as specified in Section 102370(j). Staff #1 was not listed on the facility roster. At 10:33am, LPAs called the CCL On Duty Officer of the Day and confirmed that Staff #1 was not associated to DUNN, RODEEN FAMILY CHILD CARE.

Personal Rights — Per Licensee, video footage of both the reporting party and child were in fact shared on social media disclosing the Reporting Party’s name and Child #1’s medical information.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in LPAs asked Licensee to provide the most current copy of their mandate reporter training certificate on 10/25/2023. Licensee provided LPAs with a copy of the mandated certificate with date completion of 9/10/2023 in a font different from the font on the template that was discontinued. On 10/26/2023, the mandated reporter training support department representative reviewed the certificate and disclosed in writing that the certificate submitted by licensee was not valid which poses a potential health, safety or personal rights risk to persons in care.
Advertisements and License Number — Based on observation, the licensee did not comply with the section cited above in facility's website provided by the licensee in an email dated 11/22/2023, LPAs checked to verify if the license number was listed on the advertisement and no license number was found on the website. LPAs took screen shots of both cell phone and laptop versions of the website which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in LPAs observed 4 of 4 personnel records to have AB1207 with expiration dates past the 2 year renewal requirement which poses a potential health, safety or personal rights risk to persons in care.
Family home — preventive health training, including pediatric CPR and first aid — Based on record review, the licensee did not comply with the section cited above in LPAs observed 4 of 4 personnel files with First Aid CPR certificates from a vendor that was not American Red Cross, American Heart Association or EMSA vendor which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above in LPAs observed 4 of 4 personnel files not to have yearly influenza vaccination which poses a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above in LPAs observed children's roster to be incomplete with no dates indicating end dates for care and children missing contact information which poses a potential health, safety or personal rights risk to persons in care.
