Transportation — , Based on the LPA’s conducted room observations, interviews, and review of facility records, the following findings were identified. On 7/17/2026 at approximately, 11:30 am to 1 pm, the licensee conducted a field “trip” (activities away from the center) to KidTopia Indoor Play Center at 4620 Auto Mall Pkwy Fremont CA 94538. The trip included 21 children in care (ages from 4 years old to 12 years old) with 4 staff members. The licensee provided transportation to the children from the facility to KidTopia. Staff 1 (S1) stated that they used 2 motor vehicles to transport children, however, only 1 motor vehicle had car or booster seats provided for the children. This poses a potential risk to health, safety or personal rights to children in care.
Admission Policies — , Based on the LPA’s conducted room observations, interviews, and review of facility records, the following findings were identified. On 07/16/2026 at approximately 10 am, the licensee sent a field “trip” (activities away from the center) invitation to the children’s authorized representatives, to KidTopia on 7/17/2026 at 11am. The licensee did not include transportation arrangements in this invitation. S1 stated that participation confirmation once payment ($17.00) received from each child. LPA concluded that parents (children’s authorized representatives) did not authorize transportation on this trip and as per the facility’s parents handbook, transportation is not provided to students at “our” center, signed and received by the children’s authorized representatives. This poses a potential risk to health, safety or personal rights to children in care.
