The door remained open for the children to roam through both rooms. LPA observed five children, the Director and Staff #2 to be in one room at the same time for a short period of time, leaving the other three children left unattended in the other room. Drinking water lead testing — During today's inspection, Director stated that the facility has tested their water, however did not test their water until 08/2023, which is after the due date to test the facility water of 01/01/2023. Director also stated that the facility only uses the water from the sinks for the children to wash their hands. The facility has a water service that delivers water every other week which isused for children to drink. Based on record review, the facility did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — During record review, the Director provided a document to LPA showing the last time a fire drill was conducted was on 06/25/2020. Director was unaware that fire drills need to be completed once every six months. Mandated reporter training — During record review and interviews, it was discovered that Director's mandated reporter training expired on 05/15/2021 and it has not been renewed. Also, Staff #2 has not completed the training at all in the last two years. Staff #2 began employment at this daycare in August 2023. Based on interview and record review, the facility did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
