Alterations to Existing Buildings or New Facilities(a):Prior to construction or alterations, the licensee shall notify the Department of the proposed change(s). — LPA observed a fenced playground in the parking lot and children from the Preschool playing and Director stated that she is using the indoor playground as an activity room. Director did not notify the Department of the changes made to the Facility prior to useage.
(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. — S2 and S3 have expired Mandated Reporter Training, which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation, interview and record review, the licensee did not comply with the section cited above. Director's Mandated Reporter Training expired on 8/2/2023, which poses/posed a potential health, safety or personal rights risk to persons in care.
Limitations on Capacity and Ambulatory Status — Based on observation, interview and record review, the licensee did not comply with the section cited above. Director accepted C1, who is younger than two years of age began Preschool on 5/6/2024, which poses/posed a potential health, safety or personal rights risk to persons in care.
