In reviewing the children files, LPA did not observe the required consent for emergency medical treatment (LIC 627) on file for child #6. LPA observed a Bumbo seat, without a restraint belt, which was recalled in August 2012. Although it was not use at the time of the visit, the licensee disclosed that it is used by a child in care. This poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. The licensee's current assistant had not completed/renewed the required training. This poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above. The licensee's current assistant did not have proof of required immunization (MMR/Tdap/flu) on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
