Teacher Qualifications and Duties — Based on LPA observation, staff Jaclyn Lyberger was observed supervising three toddlers children alone. Ms. Lyberger did not have units in Infant/Toddler on file at the center and is not a qualified teacher which poses/posed a potential health, safety or personal rights risk to children in care.

Limitations on Capacity and Ambulatory Status — Based on the information obtained during investigation interviews it is determined that staff S1 brought their school age child to the facility 2-4x per week over the past 2-3 months and remained in the preschool classroom for up to 8 hours which poses a potential health and safety risk to children in care.

Alterations to Existing Buildings or New Facilities — Based on observation and interviews, Toddler room 2 walls had construction without notifiying the Department which poses a potential health and safety risk to children in care.

Reporting Requirements — Based on interviews, facility failed to inform the parent of the damage sustained at the facility over the weekend and subsequently failed to inform the Department of the damage and temporary closure from 2/5 - 2/7 as a result.
