Personal Rights — … Based upon investigative interviews, four individuals stated they witnessed S1 aggressively placing an infant into a play yard and using inappropriate language on 1/8/25, which is an immediate health, safety and personal rights risk to children in care.

Infant Care Center Sleeping Equipment — … Based on information obtained from text messages, photos, and interviews with outside sources, it was determined that facility does not have an adequate amount of cribs for infants enrolled and will have infants share cribs or a play yard which is an immediate risk to the health, safety and personal rights risk to children in care.

Personal Rights — Based on the information obtained during interviews, observations, and videos it is determined that S1 violated infant C1's personal rights by handling the child in a rough manner.

Personnel Requirements — LPA observed there were no additional staff available in case of emergency and Director was required to provide AM/PM breaks as there were no additional staff available which is a potential health, safety and personal rights risk to children in care.

Staff-Infant Ratio — … Based upon staff and parent interviews conducted, 6 out of 9 staff and 2 out of 4 parents stated that the facility was out of ratio repeatedly over the course of the past 6-7 months which is a potential health, safety and personal rights risk to children in care.

Limitations on Capacity and Ambulatory Status — Based upon text messages from reporting party and interviews with staff and parents, it is determined that 5 out of 9 staff and 1 parent observed 10 infants in the infant classroom with is a potential health, safety and personal rights risk to children in care.
