Disaster and Mass Casualty Plan — Based on observation, interview and record review, the licensee did not comply with the section cited above in that there was no documentaion available for review. Per staff # 1 drill was conducted two months ago but they do not document which poses/posed a potential health, safety or personal rights risk to persons in care.
Drinking water lead testing — Based on interview and record review, the licensee did not comply with the section cited above in that Licensee stated tested has not been completed and the department has not received any documentaion which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that Staff # 1 and # 3 didn not have proof of current AB1207 certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in that Staff present had a CPR card that is not the required EMSA approved training which poses/posed a potential health, safety or personal rights risk to persons in care.
