Indoor Activity Space For Infants:The sleeping area for infants shall be physically separate from the indoor activity space. This separation shall be accomplished as specified in (b) above. Based on observation, the licensee did not comply with the section cited above. LPA observed infants in room A napping on cots/mats on the activity space near the napping room. This poses/posed a potential health, safety or personal rights risk to persons in care. Civil penalty of $250.00 is being assessed.

Indoor Activity Space for Infants — Based on observation, the licensee did not comply with the section cited above. LPA observed infants in room A napping on cots/mats on the activity space near the napping room. Also observed that the infant/toddler area did not have a separate napping area, eventually the infants/toddlers were observed napping in the activity space. This poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation, the licensee did not comply with the section cited above. LPA observed staff changing a child on a changing table that was not within arms reach of the sink. At the same time, there was another staff changing a diaper on the changing table that was near the sink. Both changing tables were located in the same space/area. This poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. In reviewing staff records/files, LPA determined that staff #1 - #3, and staff #8 did not have or had not renewed their Mandated Reporter Training. This poses/posed a potential health, safety or personal rights risk to persons in care.
Criminal Record Clearance — Based on record review and observation, the licensee did not comply with the section cited above. Staff present at the facility had a criminal record clearance but not associated to the facility license. Although the licensee has a waiver for an administrative file for staff associations, the licensee has not requested to add this facility to that waiver. This poses/posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based on interview and record review, the licensee did not comply with the section cited above in that the facility did notify the Department of the change in director which poses a potential health, safety or personal rights risk to persons in care.
