Family home — separation of children showing illness — Based on interviews conducted on 05/15/25 and 05/15/25, child C1 was excepted into care on 05/13/25 and in attendance between 7:00am and 3:48pm after vomiting during their arrival to the facility and comingled with 5 other children in care on 05/13/25, which posed a potential health and safety or personal rights risk to children in care.
Reporting Requirements — Based on LPA Strother’s interview with Licensee (L1) on 05/15/25, a child in care (C1) made claims of physical abuse to L1 on 05/13/25. L1 did not report the incident to CPS with a Suspected Child Abuse Report (SCAR) or report to Community Care Licensing Division (CCLD) as required, which posed a potential health, safety and/or personal rights risk to children in care.
