Immunizations — Based on record review, the licensee did not comply with the section cited above. Child number 2 did not have current immunization records up to date which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on interview, the licensee did not comply with the section cited above. LPAs observed that Licensee did not have an infant safe sleep log for Child number 2 in care which poses/posed a potential health, safety or personal rights risk to persons in care.
