Staffing Ratio and Capacity — Based Licensee’s handwritten declaration and interview, Licensee declared to be overcapacity on 12/30/2025, this poses a potential health, safety and personal rights risk to children in care.
Staffing Ratio and Capacity — Based on todays inspection, Licensee was found over her capacity with 7 children, and no helper. Licensee does not have a Landlord Consent on file. This is a repeat violation over the last 12 months.
Child's Records — Based on observation and interview with licensee, licensee was not able to provide children records of any of the 21 children enrolled.
Immunizations — Based on interview, licensee stated that she did not mantain immunization records for any of the children in care.
Family home — safe toys and play equipment — Based on LPAs observation, there was a broken baby gate between the Kitchen and the front entrance room. Baby gate was easily opened by children making the Kitchen (off limit area) accessible to children.
