Waivers and Exceptions for Program Flexibility — Based on records reviewed and interviews conducted, it was found that the center did not comply with its approved licensing playground waiver for McCoppin Square Park. The waiver requires that preschool children be escorted to and from the park by adult staff and that a ratio of one adult to every six preschool children be maintained during transit and while using the playground. On the date of the incident, two teachers escorted thirteen preschool children to the park, which did not meet the required one to six adult to child ratio. Failure to follow the approved waiver conditions poses/posed a potential health, safety or personal rights risk to persons in care.

101439.1 Infant Care Center Sleeping Equipment (f) Cribs shall be free from all loose articles and objects, including blankets and pillows. This requirment was not met as evidenced by: Based on observation, the center did not comply with the cited section above. On July 7, 2026, at approximately 10:05 am, LPA observed a 9 month old infant napping in a crib with a blanket covering the infant's entire body up to the neck. This poses a potential health and safety risk to infants in care.
