Infant Care Activities — Based on observation at 9:27am C1 was observed in a car seat. S1 interview stated C1 arrived sleeping in a car seat and was not moved until C1’s diaper was changed. At 11:38am infant C1 was observed sleeping on a cushion on the floor. Based on interview at 11:48am D1 stated she was planning to leave C1 sleeping on the floor, which poses an immediate health, safety or personal rights risk to persons in care.

Infant Care Activities — Based on observation at 9:27am C1 was observed in a car seat. S1 interview stated C1 arrived sleeping in a car seat and was not moved until C1’s diaper was changed. Based on interview at 11:48am D1 stated she was planning to leave C1 sleeping on the floor, which poses an immediate health, safety or personal rights risk to persons in care.

Civil penalty in addition to suspension or revocation — Based on interviews with S1 and A7, infant C1 left the preschool yard without staff supervision on an unknown date in November 2025, entering the neighbor’s yard, where the neighbor and their dog were present, which poses an immediate health, safety or personal rights risk to persons in care.

Criminal Record Clearance — Based on interview and record review, two staff S1 and S2 were working with children at the facility without an associated criminal record clearance, which poses an immediate health & safety and personal rights risk to the children in care.

Buildings and Grounds — Based on interviews and observations, the facility had dead roaches present on 05/12/26 and the licensee did not ensure that they were cleaned up before or while children were present, which poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on interviews and observations, the facility had feces present in the play yard on 05/12/26 and the licensee did not ensure that it was cleaned up before or while children were present, which poses a potential health, safety or personal rights risk to persons in care.

Limitations on Capacity and Ambulatory Status — Based on interview and record review, the facility co-mingles infant and preschool children beginning at 4:30pm for non-emergency reasons which does not follow the requirements of their waiver which poses a potential risk to the health, safety, and personal rights to persons in care.

101429(a)(2) Sleeping infant(s) shall be directly observed by sight and sound at all times. — Based on observation, an infant child (C3) was left without the supervision of a teacher in the infant room, which poses a potential risk to the health, safety, and personal rights to persons in care.

Infant Care Center Sleeping Equipment — Based on observation and interview, an infant child (C1) was sleeping with a blanket in the crib which poses/posed an immediate health, safety or personal rights risk to persons in care.

Infant Care Food Service — Based on observation and interview, D1 was observed feeding C1 while C1 was lying on a cushion on the floor and S2 stated she fed C1 while C1 was strapped in the car seat, which poses a potential health, safety or personal rights risk to persons in care.

Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation and interview, C1 was strapped in a car seat. During interview S2 stated that C1 is kept in the car seat to protect them from older infants in care, which poses a potential health, safety or personal rights risk to persons in care.

Current roster of children — Based on record review, 6 children’s birthdays missing and a dis-enrollment date for one child was not listed on LIC9040, which poses a potential health, safety or personal rights risk to persons in care.

Napping Equipment — Based on observation, 3 of the stacked 7 cots had sheets on them and were touching, which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation and record review, the swing set is intended for ages 3 to 10. 4 out of 5 preschool children were observed using the swing set and 4 out of 5 preschool children present today were under the age of 3, which poses a potential health, safety or personal rights risk to persons in care.

Infant Care Food Service — Based on observation and interview, D1 was observed feeding C1 while C1 was lying on a cushion on the floor and S2 stated she fed C1 while C1 was strapped in the car seat, which poses a potential health, safety or personal rights risk to persons in care.

During interview S2 stated that C1 is kept in the car seat to protect them from older infants in care, which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, record review and interview, multiple rodent poison bait traps were observed on the preschool playground when the playground was used by children on 11/04/25, which posed a potential health and safety risk to children in care.

Current roster of children — Based on record review the facility roster was incomplete upon request, which poses a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on interviews, licensee did not report child’s absence to the Department and did not submit a written incident report, which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on interviews, observations and records received the facility had dead cockroaches present on 09/08/25, 11/04/25 and 11/05/25 and the licensee did not ensure that they were cleaned up before or while children were present, which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on interviews, observations and records reviewed, the temperature in the classrooms felt cold and the preschool classroom was below 68 degrees on 11/05/25 and an unknown date, based on photographs of the thermostat, and today 1/23/26, which poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on interview 3 out of 6 parents reported their child having a full diaper at the time of pick up, which poses a potential health, safety or personal rights risk to persons in care.

Indoor Activity Space for Infants — Based on observation, LPAs entered the infant room observing 6 children present, 2 in the play room and 4 in the nap room, with one staff, S1, which poses a potential health & safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on observation, LPAs entered the infant room observing 6 children present, 2 in the play room and 4 in the nap room, with one staff, S1, which poses a potential health & safety or personal rights risk to persons in care.
