Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 5:12 Staff files have an expired Mandated reporter which poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher Qualifications and Duties — Based on record review, S10 was not a fully qualified teacher. S10 is missing units in curriculum.
Child Care Center Director Qualifications and Duties — Based on interviews conducted and record review, Site Director and Ass't Director leaves the preschool premises between 2:00-2:30PM. Site Director failed to designate a person in charge.
Responsibility for Providing Care and Supervision — Based on interviews conducted with staff, children on a few ocassion were unsupervised in the bathroom when a floater was not available to assist.
Personal Rights — Based on document discovered during file review, employee S9 was given a written warning discipline notice on 9/10/25 indicating that staff was witnessed to be very aggressive to some of the children. Staff was seen grabbing a child's face & pulling their arm very hard. Staff also pulled a child's head forcefully.
Reporting Requirements — Mrs. Turner failed to report to the department regarding a written warning/discipline notice that was issued to S9 on 9/10/25.
Teacher-Child Ratio — Based on observation and record review, there were 9 children in the Baby Bears room with 2 aides.
Responsibility for Providing Care and Supervision — Based on LPA interview and review of written statements by staff, the licensee did not comply with the section cited above, in that C1 was left alone, unsupervised, outside at the facility’s playground. This poses a potential health, safety, or personal rights risk to children in care.
Staff immunization requirements — Based on record review, staff S2 was missing proof of immunizations for TB and MMR which poses a potential health and safety risk to children in care.
