Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above as LPM/ LPA observed the infant play structure has spider webs, corner of the chain link fence has cluster of spider webs with dead bugs, infant toy storage box has spider webs and all play structures are dusty. The infant play structure has a metal pole protruding out of the side of the play structure which poses a potential health, safety or personal rights risk to persons in care.
Infant Needs and Services Plan — Based on record review, the licensee did not comply with the section cited above in as LPM observed 2 needs and services plan have not been updated, one needs and services plan has not been updated since 10/30/2025 which poses a potential health, safety or personal rights risk to persons in care.
Limitations on Capacity (a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. Based on observation, the licensee did not comply with the section cited above as LPM/LPA observed three preschool age children in the infant classroom; on the rug playing near and with infant toys. Mother was present in the classroom, changing infant's diaper, leaving the infant staff to provide care and supervision to the three preschool age chidlren. Per staff, this is an ongoing matter. This poses a potential health, safety or personal rights risk to persons in care.
