Food Service — Based on observation, the licensee did not comply with the section cited above as LPM and LPA observed an opened box of Trix Raspberry Rainbow Yogurt with expiration date of 07/19/2026. There were approximately 10 containers of yogurt in the open box which poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation of the outdoor preschool play yard and witness interviews, the licensee did not comply with the section cited above. The LPA observed cracked, uneven cement and protruding tree roots visibly extending out
