Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in that Child 1 (a 5 month old infant) was napping in crib inside off limits bedroom with door closed which poses an immediate health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on observation and Interview, the licensee did not comply with the section cited above in the Child #1, age 14 months was observed sleeping in car seat during visit which poses an immediate health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above in that cleaning compounds were located in kitchen cabinet under sink which posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in that 2 out of the 2 infants napping in play yard/crib had a blanket while napping which posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in that Chiod 1 (a 5 month old infant) was napping in crib which was observed to have pads on 2 sides of crib which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — LPA observed Child #1, age 13 months srtapped into car seat for about 15 minutes during this visit, child was awake, this poses a potential health, safety or personal rights risk to persons in care. This is a repeat violation - a $250 Civil Penalty is assessed
