Staff-Infant Ratio — Based on record review, the licensee did not comply with the section cited above by having 1 teacher with more than 4 infants on 4/27/26, 4/28/26, and 5/1/26, which poses an immediate health, safety or personal rights risk to persons in care.

Staff-Infant Ratio — Based on LPA observation and interviews conducted, the facility is operating out of ratio repeatedly during drop off and nap time. The facility did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

101416.5 Staff-Infant Ratio (b) There shall be a ratio of one teacher for every four infants in attendance. This requierment was not met evidence by: LPAs observed two occassions where one staff member was left to supervised more than 4 infants in care which poses an immediate health and safety risk to the children in care.

1596.956 Child day care centers serving infants; optional toddler program...(a) The department shall develop guidelines and procedures...(6) The toddler program shall be conducted in areas separate from those used by older or younger children... — Based on staff interviews that were conducted LPAs confirmed children enrolled in the toddler program are commingling with the infant classroom, the facility did not comply with the section cited above in which posed a potential health,and safety risk to persons in care.

101429 Responsibility for Providing Care and Supervision for Infants (a) In addition to Section 101229, the following shall apply: (2) Sleeping infant(s) shall be directly observed by sight and sound at all times. This requierment was not met as evidence by: Based on LPAs observation of staff seated on the floor with 3 children, 1 staff with one child on the diaper changing station and 2 sleeping infants in the nap room and the lack of documentation required for the sleep log, facility is not providing adequate visual and auditory supervision.

Facility is operating out of ratio.

Staff-Infant Ratio — LPA Clayton observed the Teacher - infant ratio to be out of regulatory compliance (1 Teacher caring for 5 infants), and LPA Wheatley observed the Teacher – infant ratio to be out of regulatory compliance (2 teachers caring for 16 toddlers) posing an immediate Health and Safety risk to children in care.

Limitations on Capacity and Ambulatory Status — LPA Clayton observed 12 chidren (aged 2 years) and five 4 year old children in the same classroom, being supervised and instructed by the same staff, which poses and immediate Health and Safety risk to children in care.

Responsibility for Providing Care and Supervision — LPA Clayton and LPA Wheatley observed chidren on the play yard out of the visual supervision of the teacher and having to remind her tc bring the chidren to where she could see them, posing an immediate Health and Safety risk to children in care.

Responsibility for Providing Care and Supervision — Based on observations of the toddler classrooms, the licensee did not comply with the section cited above in 3 of 3 instances which poses a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on records reviewed, an unusual incident was not reported;the facility did not comply with the section cited above in 1 of 1 incident that occurred on 6/30/26, which poses a potential health, safety, or personal rights risk to persons in care.

Buildings and Grounds — Based on interviews, a child's clothing got discolored after being exposed to remnants of bleach water solution, the facility did not comply with the section cited above in 1 of 12 toddler children, which poses a potential health, safety, or personal rights risk to persons in care.

Reporting Requirements — Based on interviews, an incident was not reported to the child's authorized representative until they asked about it at pickup. The facility did not comply with the section cited above in 1 of 12 toddler children, which poses a potential health, safety, or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants — Based on record review, the licensee did not comply with the section cited above in 5 of 19 infant sleep checks, which poses a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on interviews, the facility did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Infant Care Activities — Based on LPA observation of 2 infants using sleep sacks, and interviews revealed 6 of 8 infants in the Infant 1 room use sleep sacks, the facility did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants — Based on record review and interviews, documentation of 15 minute sleep checks were not maintained and available for review; the facility did not comply with the section cited above with 5 of 8 infants which pose a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, the facility did not comply with the section cited above LPAs observed cleaning solutions in an unlocked cabinet underneath the children's sink. Children were not present during the observation in which posed a potential health,and safety risk to persons in care.

101221 (a) A separate, complete and current record for each child is maintained in the child care center. This is evidenced by: LPA Wheatley requested Child #1 file and was unable to review because the child does not have a complete file. This is a potential risk to the heatlh & safety of children in care.

101216.1(c) - To be a fully qualified teacher, a teacher shall have one of the following: Twelve post-secondary semester or equivalent quarter units in early childhood education or child development completed, with passing grades, at an accredited or approved college or university; and at least six months of work experience in a licensed child care center or comparable group child care program. This is evidenced by: LPA Wheatley reviewed staff records and observed several staff supervising without units. This is a potential risk to the heatlh & safety of children in care. — requirement not met
