In touring the back yard, LPA observed two swing sets/structures that were not anchored. At this time, due to extreme weather, children are not participating in outdoor play. Therefore, this poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above. Based on the fire/disaster drill log observed, the last drill conducted was on 7/27/2023. This poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Children #2 and #3, did not have complete files for review. Files were missing Identification (LIC 700) and Medical consent (LIC 627) forms. This poses/posed a potential health, safety or personal rights risk to persons in care.
