Personal Rights — Based on interviews conducted there was a personal rights violation. This is an immediate risk to the health and safety of children in care.

Mandated reporter training — Based on record review and interview, the licensee did not comply with the section cited above in that 1/5 staff (Staff 2 [S2]) did not complete renewal of mandated reporter training. S2 did have proof of completion of a different mandated reporter training certificate, however this certificate did not satisfy the requirements created by Assembly Bill 1207, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review and interview, the licensee did not comply with the section cited above in that 2/5 staff (Staff 4 [S4] & Staff 5 [S5]) did not have current EMSA approved Pediatric CPR/First Aid on file while S4 and S5 transported children alone, which poses/posed a potential health, safety or personal rights risk to persons in care.
Written directives for drinking water lead testing — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed during record review the facility was not cited for a lead exceedance in 2022/2023. Director stated a retest of outlet identified with lead is being conducted.
