During the staff file review, LPA observed S2, S4, S7, S8, S9 are missing proof of TDap, S3, S8, S9 are missing proof of measles vaccine, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on observation, interview, and record review, the licensee did not comply with the section cited above as in not all napping bedding is individually separated for children in care, which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observations and records review licensee did not comply with the section cited above in 4 out of 5 records Mandated Reporter Certificate was expired since 07/2023 which poses a potential health, safety or personal rights risk to persons in care.
