Criminal background check — Based on observation and record review, the licensee did not comply with the section cited above in one count out of two staffs' files missing association of fingerprinting clearance for Sandy Phakornkham which poses a potential health, and safety risk to persons in care. Sandy has been working at the facility since February 2026 with clearance ID number on file. However is not associated to the facility as it is required. An immediate civil penalty of 500.00 was assessed today.
