Fixtures, Furniture, Equipment and Supplies — The licensee did not ensure playground equipment was safe and free of sharp pointed parts. Based on interviews and observation, the playhouse base contained a sharp, pointed screw causing injury to C1. This posed an immediate threat to the health and safety to children in care.

Limitations on Capacity and Ambulatory Status — , Based on LPAs observations it was witnessed on 2/19/26 2 preschool-aged children in the infant room. Based on interviews conducted it was stated that children from the 2 year old classroom will sometimes be in the infant room in the morning until the next teacher arrives. This is a potential health and ssafety risk to children in care.

Teacher Qualifications and Duties — Based on interview and record review, the licensee did not comply with the section cited above in S1 was left alone in classroom with children but does not have 12 ECE units which poses/posed a potential health, safety or personal rights risk to persons in care.

Admission Policies — , Based on record review and interviews conducted, authorized pick up was not notified of any potential injury until after C1 was released from the facility and the authorized pick up noticed a cut and blood on C1s head. This is a potential risk to the healthand safety of children in care.

Personal Rights — , Based on video observation and interviews conducted, cots were propped up against the wall and fell onto C1 resulting in a head injury that required medical attention. This is a potential healthand safety risk to children in care.

Personal Rights — , Based on interviews conducted it was disclosed that S1 has been observed to pulling children by their arm in a rough manner and tugging on their hair to move them into line and has been observed and overheard yelling or raising voice in classroom towards children. This is a potential health and safety risk to the children in care.

Personal Rights — , Based on interviews conducted, it was disclosed that S1 would mock the children and toss items such as plates towards them on the table during meal time. This is a potential health and safety risk to the children in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 2 out of 4 persons missing proof of vaccines in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in 2 out of 4 persons missing physical & TB test in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 2 out of 6 persons missing physical in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 2 out of 6 persons missing TB clearances in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on interview and record review, the licensee did not comply with the section cited above in children were alone with teachers aides which poses/posed a potential health, safety or personal rights risk to persons in care.

Personal Rights: (a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. This was not met as evidenced by, Based on staff interviews and video surveillance, it was disclosed and observed that S1 grabbed C1 in an inappropriate manner. This is a potential risk to the health and safety of children in care.
