101439: Infant Care Center Fixtures, Furniture, Equipment and Supplies: (g) "Furniture shall be maintained in good repair and safe condition." This requirement is not met by: Based on observation and interview, Facility did not comply with this section cited above as LPA’s observed a larger crib in classroom 5 with an infant child inside of the crib with the latch lock faced forward and partially unlocked, which poses a potential Health, Safety or personal rights risk to persons in care.
101439.1:Infant Care Center Sleeping Equipment: (f) "Cribs shall be free from all loose articles and objects, including blankets and pillows." This requirement is not met by: Based on observation and interview, Facility did not comply with this section cited above. At approximately 9:15am, in the back side of classroom 5/8 (sleeping room), LPAs observed a pink blanket in one of the infant cribs with no infants present, which poses a potential Health, Safety or personal rights risk to persons in care.
101429(a): Responsibility for Providing Care and Supervision for Infants: (2) "Sleeping infant(s) shall be directly observed by sight and sound at all times...(C) Documentation shall be maintained... and be available to the Department for review. Documentation shall include the following:1. Date, 2. Infant’s name,.3. Time of each 15-minute check, 4. Initials of staff person who conducted each check." This requirement is not met by: Based on observation, interview, and record review, Facility did not comply with this section cited above as Sleep logs that were provided to LPA’s were from March 2024 and prior which poses a potential Health, Safety or personal rights risk to persons in care.
101416.3(b): Infant Care Aide Qualifications and Duties: "An infant care aide shall work under the direct supervision of the director, the assistant director or a fully qualified teacher, except as provided for in Section 101416.5(d)(1)." This requirement is not met by: Based on observation and interview, Facility did not comply with this section cited above. At approximately 9:35am, LPA’s observed an unqualified staff alone with a group of 3 infants leave the classroom which poses a potential Health, Safety or personal rights risk to persons in care.
