Frye Family Child Care

Personnel Requirements — Based on interviews and record review 1 of 5 staff did not obtain criminal record clearances prior to working with the children for more than 5 days. This poses an immediate risk to the health and safety of the children in care. A civil penalty of $500.00 will be issued on the LIC 421BG.

Personal Rights — Based on interviews with four (4) of five (5) interviews revealed that when children exhibited challenging or unwanted behaviors staff would place children in high chairs for an undisclosed amount of time to control the child’s behavior. This poses a potential health and safety risk to children in care.

Staffing Ratio and Capacity — Based on interviews and records reviewed one staff would leave the family child care home to pick up school-age children leaving one staff to supervise over eight children at one time. This poses a potential health and safety risk to children in care.

Personal Rights — Based on interviews and records reviewed staff would leave children in swim diapers, put children in diapers not belonging to the child causing allergic reactions, or diapers were placed on backwards. This poses a potential health and safety risk to children.

Family home — food storage and labeling — Based on interviews, and observations bottles and cups and food are not labeled with the child’s name to identify that food belongs to the child the food was brought for. This poses a potential health and safety risk to children in care.

Staffing Ratio and Capacity — Based on observation and record review, the licensee did not comply with the section cited above in 5 out of 12 children were infants, which posed a potential health, safety or personal rights risk to persons in care.

102416.5 Staffing Ratio and Capacity (b) For a Small Family Child Care Home, the maximum number of children for whom care may be provided at any one time, including children under age 10 who reside at the licensee's home, shall be one of the following:(1) Four infants; or This was not met as evidence by.... Based on interviews and observation Licensee has more than the alotted amount of children in care due to having four infants. This poses a potential health and saftey risk to children in care.

Personal Rights — Based on confidential interviews 2 of 2 interviews disclosed children were picked up in soiled diapers and parents are not providing diapers as often as they probably should. Interviews also revealed P2's infant came home often with rashes which improved once the infant was removed from LICs care. This poses a potential health, safety or personal rights risk to persons in care.

102417 Operation of a Family Child Care Home (g) The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not be limited to: (3) Where children less than five years old are in care, stairs shall be fenced or barricaded. Based on observation on 05/23/2024 Licensee has placed a barricade to prevent access to the second floor. This posed a potential health and saftey risk to children in care.

102417 Operation of a Family Child Care Home (g)The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not be limited to:(4)Poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children shall be stored where they are inaccessible to children. This evidence has not been met by.... Based on observation and confidential interviews staff typically leave cleaning supplies out due to cleaning throughout the day. LPA's also observed cleaning solutions underneath the accessible kitchen sink as well as next to the FCCH changing station. This poses a potential health and safety risk to children in care.

102425 (c) Infant Safe Sleep An Individual Infant Sleeping Plan LIC 9227 (3/20)] shall be completed for each infant up to 12 month of age the provider has in care and maintained at the facility in the infant’s file. this was not met as evidenced by.... Based on record review and interview with licensee, one infant did not have an infant safe sleep plan on file at time of inspection. This poses a possible health safey and personal rights risk to children in care.

102425 (j)(D) INFANT SAFE SLEEP; The provider shall supervise infants while they are sleeping and adhere to the following requirements: Documentation shall be maintained in the infant’s file and be available to the Department for review. Documentation shall include the following: Date, infant’s name, time of each 15-minute check.This was not met as evidenced by.... Based on record review and interview, the licensee did not have infant sleep log for four infants in care at the time of inspection. This poses a possible health safey and personal rights risk to children in care.

102416.1 (d)Personnel Records; All personnel records shall be maintained at the child care home and shall be available to the licensing agency for review. This was not met as evidenced by... Based on record review and interview licensee does not have personnel records for two out of two staff present at time of inspection. This poses a possible health safey and personal rights risk to children in care.

102421(a) Child's Records; The licensee shall maintain, in each child's record, the signed and dated notice form required in Section 102419(d).this was not met as evidenced by.... Based on Record review and interview, licensee stated she has the files online, was not able to present all records at time of inspection for three out of six children present at time of inspection. This poses a possible health safey and personal rights risk to children in care.

102418(a) Immunizations; Prior to admission to a family day care home, children shall be immunized against diseases as required by the California Code of Regulations, Title 17, beginning with Section 6000. This was not met as evidenced by... Based on record review and interview, licensee did not have immunization records for four out of six children in care at the time of inspection.
