In reviewing staff files/records, LPA observed and determined that the TB test for staff #1 was taken in 2007 however, their hire date was in 2023. This poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S2 was missing proof of the Mandated Reporter Child Care Providers Training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S6 was missing proof of the MMR vaccine which poses a potential health, safety or personal rights risk to persons in care.
